Causal characterization of functional connectivity through the spread of electrically induced oscillations in the epileptic human brain

Little is known about the rules governing the spread of local entrainment within synchronized networks distributed across the brain. The assessment of the causal influences impacting information flow between two brain regions have mainly relied on confirmatory model-driven approaches (such as dynamic causal modeling and structural equation modeling) and exploratory data driven approaches (such as Granger Causality analysis). However, stimulation-driven approaches offer a unique opportunity to impact ongoing brain activity and describe the causal consequences of such manipulations, performed on a specific node of a complex cerebral network. In this project, we characterize causal functional interactions between brain regions by assessing how frequency-tuned electrical currents delivered intracranially in awaken epileptic patients enhance inter-regional synchrony between pairs of areas. To achieve this goal, we worked with an existing iEEG database from 18 medication-resistant epilepsy patients undergoing Intracortical Stimulation Mapping Procedures (ISMP) performed to causally identify and localize the epileptogenic foci, prior to neurosurgical removal. Patients are implanted with series of multi-electrodes in well-known brain regions under MRI guidance. Intracranial EEG contacts allow continuous recordings and the delivery through pairs of adjacent contacts of biphasic pulses of rhythmic Direct Electric Stimulations (DES) at a 50Hz frequency coupled to electrophysiological recordings. Measuring significant increases in gamma power ( 50Hz) observed during the stimulation period (vs. prior the stimulation), and significant increases of Phase-Locking Value (PLV) between signals recorded in the electrically stimulated regions and activity evoked in the rest of implanted regions during stimulation (vs. prior simulation), we characterize the spread of oscillatory entrainment from the stimulated region to the remaining regions, thus establishing a network of functional connectivity in the brain. By comparing this network with the one shown during resting-state, we assess how entrainment to frequency-tuned electrical currents delivered intracranially is predicted by the resting-state functional connectivity network

An integrated platform for the support of teaching activities as been developed and deployed at the Aveiro Norte Polytechnic School of the University of Aveiro. In this paper we present an approach to Learning Roadmaps for Higher Education based on this platform. The aprend.e platform – Electronic Integrated System for Learning and Training - has at its core a Learning Management System with a number of plugins. It represents a new challenge for the University of Aveiro for higher education and is already being at its core is the concept of learning roadmaps that act upon two fundamental axes: education and learning. For the teachers, it aims at becoming a self-supporting tool that stimulates the organization and management of the course materials (lectures, presentations, multimedia content, and evaluation materials, amongst others). For the students, the learning roadmap aims at promoting self-study and supervised study, endowing the pupil with the capabilities to find the relevant information and to capture the concepts in the study materials. The outcome will be a stimulating learning process together with an organized management of those materials

    Get PDF
Conclusões: O pico de VO2 e o índice de eficiência respiratória VE/VCO2 estão intimamente associados à resposta clínica e ecocardiográfica à TRC, com valores mais favoráveis nos doentes com resposta à TRC.Background: Although most of the patients treated with cardiac resynchronization therapy (CRT) respond to the therapy, 30 to 35% do not experience clinical improvement. The best criteria to evaluate response remain controversial. The cardiopulmonary exercise testing (CPX) allows an objective analysis of the functional, ventilator, cardiac and metabolic response during effort. Objectives: Analyze and characterize ergometric variables obtained during CPX in a population of patients with CRT, evaluate the association of those variables after CRT implantation with the definition of response and analyze the association between the response to CRT with mortality and heart transplant. Methods: Seventy nine patients with CRT who underwent CPX were studied. An improvement of ≥1 NYHA class defined clinical responders; a ≥25% improvement in left ventricular ejection fraction (LVEF) identified echocardiographic responders. We evaluated demographic, electrocardiographic, echocardiographic and clinical parameters before and after CTR implantation, as well as ergometric parameters after TRC implantation. It was made a statistical study to analyze the association between variables before and after TRC, studying their differences according to the response definitions and their ability to predict response to CRT. Results: There were 65,6% clinical responders and 40,5% echocardiographic responders. The differences observed in exercise tolerance between clinical responders and non responders were statistically significant (peak VO2 of 17,9 ± 5,1 mL.kg-1.min-1 versus 14,8 ± 5,0 mL.kg-1.min-1 (p=0,022); VE/VCO2 slope of 23,7 ± 15,5 versus 36,1 ± 17,1 (p=0,009); O2 pulse of 12,0 ± 3,3 versus 10,0 ± 3,3 (p=0,025)); there were observed important differences in mortality between both subgroups. (4,8% versus 18,2%, p=0,080). VE/VCO2 slope was the best to predict response to CRT, independently of the definition of response used (0,754 (p=0,007) e 0,790 (p=0,001)). O2 pulse and peak VO2 presented inferiors areas under the ROC curves (0,726 e 0,740 versus 0,708 e 0,693, respectively). Base VO2 was the best mortality predictor (0,784 (p=0,025)). Peak VO2 was the best predictor for the evaluation of patient’s need of heart transplant. (0,688 (p=0,030)). Conclusion: Peak VO2 and VE/VCO2 slope are inwardly associated with clinical and ecocardiographic response to CRT, with more favorable values on responders to CRT

The Portuguese justice in the Middle Ages (An inventory of doubts)

This paper tries to raise some of the main questions about the way of the portuguese judicial system worked in the Late Middle Ages. The first one concerns the effectiveness of the laws (specially the Ordenações Afonsinas). Then we examine the lay landlord's justice, the conflicts and cooperation between that and the king's justice, the ecclesiastical justice, trying again to establish the part of the oppositions and the part of complementarities, to end with the local justice and in particular the local judges their profile, their cultural and juridical preparation. A last question regards the relations between those different judicial systems and the non linear evolution of the whole judicial world
